Objective:To determine whether tadalafil, a phosphodiesterase-5 (PDE-5) inhibitor, improves hemodynamics and efficacy of cialis in patients with erectile dysfunction (ED).
Methods:A prospective, open, blinded, controlled study of patients admitted to our outpatient clinic for ED between April 2006 and July 2012 was conducted at our hospital. Inclusion and exclusion criteria were as follows: patients with a diagnosis of erectile dysfunction (ED) or moderate to severe impotence, and patients with the following characteristics: male factor or organic causes, erectile dysfunction is accompanied by erectile dysfunction, and the patient is on tadalafil (Tadalafil) for 4 weeks. Patients with a history of tadalafil use and/or taking nitrates, including nitroglycerin or amyl nitrite, were excluded. Patient and investigator consent was obtained from all patients, and all data were reviewed for completeness. Clinical evaluation was performed before enrollment and up to 4 weeks after the last dose of tadalafil. After the first dose, patients were asked to complete a modified International Index of Erectile Function (IIEF), Erectile Function Index (EFI), and Sexual Encounter Profile (SEP) questionnaire to assess the erectile function, and then to complete a question about the frequency of sexual intercourse.
Results:In this study, we found that the mean age of patients with ED improved by 4.5% after first dose of tadalafil compared with 4.6% after one dose of tadalafil. The mean score on the IIEF and SEP was 4.3 and 3.5, respectively. No significant difference was observed between patients and patients with tadalafil or placebo groups in terms of age, IIEF score, and SEP scores. After tadalafil, there were no significant differences in the score of the SEP on either of the IIEF or SEP questionnaire between patients and patients with tadalafil or placebo groups. The mean total IIEF score improved by 4.2 points compared with 4.4 points for patients with tadalafil (P<0.001). The mean total SEP score improved by 4.3 points compared with 4.1 points for patients with tadalafil (P<0.001). There were no significant differences between patients and patients with tadalafil or placebo groups in terms of IIEF, SEP, or EFI score.
Conclusion:Tadalafil is a useful tool in the treatment of ED in patients who are on tadalafil for 4 weeks. It is safe, effective, and well-tolerated. The study of this drug should be part of a broader study of tadalafil therapy in ED and related conditions.
